Please complete one form per child in BLOCK CAPITALS
You only need to complete this form once each school year.
To book your child's place at X:site please complete this form and return it to us as soon as possible and if you do, you will be able to get into the next event quickly and easily, so go ahead and fill it in right now.
Heres what to do:

Print it out
Fill it in (don't forget to include your friends names if you want to be in the same group!)
Get a grown-up to sign it
Post it back to us, using the address on the form
Then just turn up on the night and be prepared for loads of fun!
What does it cost?
To help us be able to put on an event like X:site, we ask that people pay £2.50. But if you really don't have that money then just let us know and we can use our grant fund to cover your cost.
Please make cheques payable to "Scripture Union" and send it in with your registration form.
